Just an FYI,
I received this notice from my utility (PG&E) about a Square D Brand General Duty Safety Switch 30/60A, 120/240Vac, NEMA 3R recall.
Some date code/models do not open the internal knife switches when the outside handle is thrown. Apparently (reading the recall notice), if the switch does function correctly (switches open when outside handle is moved to OFF), then all is OK.

Inspection procedure regarding the following catalog numbers manufactured within date codes 1401 – 1803 (January 1,
2014 – January 18, 2018). The safety switch can potentially remain energized when the handle is in the "OFF" position.
This poses a potential risk of an Electric shock or Electrocution hazard to customers. Refer to Table 1: Impacted

Yes, this model was used for my GT System ac disconnect. However, my system is way older than these date codes.
