Grundfos CIU 903 - No contact to SQ Flex error

I have a newly installed CIU 903 on a 11 SQF-2 and I am getting a "No contact to sq flex" error at the controller. The pump runs continuously when it has power. I'm running it off of a 120 VAC generator. I measured voltage at pins 1 & 2 on the IO module and it's 122 VAC. Does the controller need to be programmed to work with the 11 SQF-2? Perhaps the distance from the pump to the controller is an issue? I have the controller installed about 250 ft. away from the well using 10 ga. wire. The generator supplies power at the location of the controller as well.

Update: I moved the controller directly to the well head so the distance is as short as possible. Same result - no communication.
